Care Coordination

  • Care Coordination Resource of Alaska (CCRA) is here to help you navigate the community services in your area, assisting you through the maze of health care, financial and social services available in your chosen community.

  • Our Care Coordination Program is a partnership process that uses our Care Coordinator’s expertise in identifying, assessing, planning, monitoring and finding the best services in the community to meet the needs of individuals regardless of age, income or disability.

  • CCRA’s Care Coordinators are all certified and possess the education, knowledge, skills and experience needed to work with you and your chosen planning team, helping you reach your full potential. Through a collaborative effort of service providers, health care professionals and your support system we work to keep you in your chosen home for as long as possible.

  • We believe in person-centered care, being your advocate and adhering to ethical, legal, regulatory standards and guidelines.

  • Our staff of over 20 care coordinators serve:

    • Elders needing assistance

    • Alaskans with Alzheimer’s Disease and Related Dementias

    • People with Intellectual and Developmental Disabilities

    • Children with Complex Medical Conditions

    • Adults with physical disabilities
